Uses and Disclosures

Confidentiality
Therapy services are confidential, with standard legal exceptions. These limits are explained at the beginning of services.
Forensic Services
Forensic and evaluation services are not confidential in the same way as therapy. Information may be included in reports, shared with attorneys or courts, and used in legal proceedings.
Your Rights
You have the right to access your records, request corrections, and receive confidential communications. Additional rights may apply depending on the nature of services.
Provider Responsibilities
LJC Psychological Services Group is required to maintain the privacy of your information and provide this notice.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with the practice or with the U.S. Department of Health and Human Services.
